Job summary

FOX Rehabilitation in Park Ridge, IL is seeking licensed Physical Therapists to deliver one‑on‑one care to older adults in private residences and senior living communities through our Geriatric House Calls model. Qualifications

• Licensed PT with active IL license or eligibility

Responsibilities

• Provide one-on-one PT to older adults in home or community settings
• Perform evaluations and develop individualized plans of care
• Deliver evidence-based treatment sessions
• Document in FOX EMR per regulatory standards
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers and staff on safety and function
• Offer telehealth/virtual care when appropriate to extend access
