Job summary

FOX Rehabilitation in Round Rock, TX seeks a licensed Physical Therapist to deliver one-on-one geriatric care in homes and senior living settings. You will assess, plan, and implement evidence-based therapy while building strong patient relationships and autonomy over your schedule.

You'll collaborate with an interdisciplinary team, document in the EMR, and may incorporate telehealth to expand access and continuity of care. New graduates welcome to apply for mentoring programs.

Qualifications

• Valid Physical Therapy license in the state(s) of practice.
• Degree from an accredited Physical Therapy program.
• Current CPR certification.
• Basic computer and EMR proficiency.

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home (private residences, senior living communities, etc.).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, mobility, and independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access and flexibility.
