Job summary

Lightheart Mental Health, part of Transformations Care Network, seeks licensed or licensure-eligible therapists to provide outpatient individual and family therapy. You will work within a multidisciplinary team to deliver compassionate, evidence-based care and support licensure progress.

Qualifications

• Possess a Master's degree in Social Work, Counseling, Marriage and Family Therapy, or a related field.
• Passionate about mental health and committed to providing high-quality care.
• Dedicated to ongoing professional development including supervision and continuing education.

Responsibilities

• Conduct culturally sensitive, strength-based assessments for new clients and families, creating a welcoming environment for diverse backgrounds.
• Provide outpatient individual and family therapy based on each client's needs.
• Develop crisis, safety, relapse prevention, and wellness plans to support stable functioning.
• Participate in case consultations, agency meetings, and discharge planning.
• Ensure billing, documentation, and treatment plans are completed timely.
• Engage in collaborative care with psychiatrists, nurse practitioners, dietitians, administrative staff, and fellow therapists.
