Job summary

Lightheart Mental Health, part of Transformations Care Network, seeks an Associate Mental Health Therapist to join a collaborative clinical team. You will deliver outpatient therapy, participate in case consultations, and support clients toward growth and resilience.

• Master's degree in Social Work, Counseling, Marriage and Family Therapy, or a related field.
• Passionate about mental health and committed to high-quality care.
• Ongoing professional development including supervision and continuing education.

Responsibilities

• Conduct culturally sensitive assessments for new clients and families.
• Provide outpatient individual and family therapy based on client needs.
• Develop crisis, safety, relapse prevention, and wellness plans.
• Participate in case consultations, agency meetings, and discharge planning.
• Ensure timely billing, documentation, and authorization forms.
