Comagine Health is seeking an Associate Improvement Advisor to support operational and administrative activities within the HSI Utah HUB contract. The role starts with project coordination and administration, then grows toward referral coordination for the Hub, offering a clear growth path for early-career professionals.

Responsibilities

• Support UTAH HUB operational workflows and CMS forms processing.
• Maintain records, tracking systems, and internal databases.
• Coordinate communications with dialysis facilities, CMS contacts, vendors, and partners.
• Route communications via shared phone, email, and virtual fax systems.
• Assist with facility onboarding, closures, and related processes.
• Prepare reports, reminders, and meeting materials.
