Job summary

NeighborHealth is seeking an Assistant Center Manager for the PACE Center, based at 10 Garofalo Street, Revere, MA. You will coordinate the interdisciplinary team, support care planning, and ensure regulatory compliance to deliver quality care.

Under the Director of Administration and Operations, you will assist in day-to-day operations, oversee enrollment activities, and help maintain biannual care plan reviews consistent with PACE requirements.

Qualifications

• College degree in a related health care field.
• Supervisory and administrative experience preferred.
• Experience in a managed care setting preferred.
• Experience in interdisciplinary team setting.

Responsibilities

• Oversees the Care Planning process including maintaining biannual review schedules.
• Oversees Service Determination Request process ensuring regulatory compliance.
• Monitors compliance with discipline assessments and Participant Care Plan reviews.
• Assists IDT members with coordination family meetings and enrollment signings.
• Assumes responsibility for PACE center management and oversight in the absence of the Center Manager.

The Assistant Center Manager provides coordination and planning support to all Interdisciplinary Team (IDT) members that represent the various PACE service areas and clinical disciplines. Will also provide enhanced planning and coordination to IDT care services. The Assistant Center Manager will be well versed in regulatory requirements related to PACE, the IDT, assessments, and care planning to ensure that the model of Team Based Care provided is applicable with policies and procedures. The Assistant Center Manager, in partnership with the PACE Center Manager, reviews and analyzes discipline specific assessment information to ensure compliance with PACE regulations. The Assistant Center Manager coordinates team meetings to facilitate participant planning and Care Plan creation. The Assistant Center Manager will assume responsibility for PACE facility management and oversight in the absence of the Center Manager and assist in day-to-day operations as assigned.Assistant Center Manager tasks:• Oversees the Care Planning process including maintaining biannual review schedules.• Oversees Service Determination Request process ensuring regulatory compliance• Monitors compliance with discipline assessments• Monitors compliance with Participant Care Plan reviews• Assists members of the IDT with coordination family meetings• Ensures/Oversees compliance with Care Plan goals/dates• Assists with meeting facilitation upon Center Manager direction/request, including morning meeting and IDT Care Plan meetings• Assists with Enrollment signings• Runs, distributes, maintains reports determined essential to care coordination Assumes responsibility for PACE center management and oversight in the absence of the Center Manager Assists in the day-to-day operations as assignedEducation:College degree in a field such as Rehabilitative Therapy, Nursing, Social Work, Recreational Therapy, Health Care Administration, or related work experience is required.Experience:Supervisory and administrative experience is preferred.Skilled facilitator with excellent communication and interpersonal skills.Experience in a managed care setting preferred.Prior experience in interdisciplinary team setting.Schedule: Monday through Friday 8am to 4:30pm (includes a 30 minute lunch break)Salary range: $60,840 up to $91,520 based on experience.EEO & Accommodation Statement:NeighborHealth is an equal employment/affirmative action employer.
