Job summary

NeighborHealth in the Greater Boston area seeks a Per Diem Personal Care Assistant to cover day room staff absences at the PACE Center. You will assist participants with personal care, meals, and activities while supporting safe transportation and day-center operations.

You may float to other sites to support patient care, and you will report to the Activities Coordinator and PACE nurses. A High School diploma or GED and a CNA or Home Health Aide certification are required; bilingual abilities

Qualifications

• High School graduate or GED.
• Certification of Nursing Assistant or Home Health Aide.
• Experience with older adults or frail populations preferred.

Responsibilities

• Assists PACE Center participants with personal care, restorative rehabilitation, meals, group and individual activities.
• Accompanies participants on transportation as needed to ensure safety.
• May be assigned to assist supported housing participants with morning care.
• Actively contributes to the operations of the Day Center by contributing ideas for the activities program and offering suggestions to improve daily operations.
• Contributes to participant care planning and assessment through reports to the Activities Coordinator, PACE Center Nurses and Rehabilitation staff.
• Requires employee to float to other sites to support patient care needs across the program.
