Job summary

WellBridge Healthcare Fort Worth is seeking an Assessment Specialist RN to join our team in a full‑time days schedule with three 12‑hour shifts per week. You will lead initial assessments, coordinate referrals, and collaborate with MDs for LOC determinations, while ensuring accurate preadmission and insurance paperwork.

Requirements include an RN license, an Associate's Degree in Nursing (or a Master’s in social work/counseling), CPR, and CPI within 30 days.

Qualifications

• Associate's Degree in Nursing or Master's in social work/counseling required.
• Current RN license as required by state regulations.
• CPR Certification and CPI within 30 days of hire.

Responsibilities

• Facilitates response to service requests, including initial response, assessment and referral.
• Documents patient symptomology, functioning and needs; assists with LOС determination.
• Maintains preadmission and admission paperwork; completes pre-certification with insurers.

How You'll Contribute

An Assessment Specialist RN who excels in this role:

• Facilitates response to a request for services, including the initial response, the assessment, and referral process, the designation of the appropriate level of care, admission into services, and appropriate follow up activity
• Conducts initial evaluation, initiates the assessment, documents patient symptomology, functioning, and needs and provides information to MD for LOC determination
• Demonstrates excellent customer service skills in assisting patients, physicians, visitors, and coworkers in a prompt and courteous manner
• Possess clear knowledge of Emergency Detention paperwork and processes and EMTALA
• Maintains all preadmission and admission paperwork with accuracy, and completes all pre-certification with insurance companies with insurance companies

What We're Looking For

Applicants should have an Associate's Degree in Nursing or Master's degree in social work or counseling required. Additional requirements include:

• Current clinical, social work or RN license as required by state regulations
• Prior experience in s psychiatric healthcare facility working with adult and geriatric patients preferred. Must have strong clinical assessment skills.
• CPR Certification and Crisis Prevention Training (CPI) within 30 days
