Job summary

WellBridge Healthcare Fort Worth is seeking an Assessment Specialist RN to join our team on a PRN basis in Fort Worth, Texas. You will manage the initial assessment, referral and follow-up, coordinate care levels, and assist with insurance pre-certifications, with a focus on compassionate, timely patient care.

The role requires an Associate's Degree in Nursing or a Master’s in social work/counseling, current RN license, CPR, CPI within 30 days, and experience in psychiatric care for

Qualifications

• Requires RN licensure or relevant licensure per state regulations.
• Experience in psychiatric healthcare with adults/geriatric patients preferred, strong clinical assessment skills.
• CPR certification and CPI within 30 days of start.

Responsibilities

• Facilitates response to service requests, initial assessment, referral, and follow-up.
• Documents patient symptomatology, functioning, needs, and assists MD with LOC determination.
• Provides excellent customer service to patients, physicians, visitors, and coworkers.

How you'll contribute

An Assessment Specialist RN who excels in this role:

• Facilitates response to a request for services, including the initial response, the assessment, and referral process, the designation of the appropriate level of care, admission into services, and appropriate follow up activity
• Conducts initial evaluation, initiates the assessment, documents patient symptomology, functioning, and needs and provides information to MD for LOC determination
• Demonstrates excellent customer service skills in assisting patients, physicians, visitors, and coworkers in a prompt and courteous manner
• Possess clear knowledge of Emergency Detention paperwork and processes and EMTALA
• Maintains all preadmission and admission paperwork with accuracy, and completes all pre-certification with insurance companies with insurance companies

What we're looking for

Applicants should have an Associate's Degree in Nursing or Master's degree in social work or counseling required.

Additional requirements include:

• Current clinical, social work or RN license as required by state regulations
• Prior experience in s psychiatric healthcare facility working with adult and geriatric patients preferred. Must have strong clinical assessment skills.
• CPR Certification and Crisis Prevention Training (CPI) within 30 days
