Winona Health in Winona, MN, is seeking an Assessment Nurse for Lake Winona Manor. This 1.0 FTE role runs days 7:30am-5pm with weekends every 4th and holidays as specified.
The nurse will assess, evaluate, and act to improve quality of care for long-term residents and mentor the care plan process. Responsibilities include MDS-related assessments, holistic resident evaluation, QA participation, and ensuring documentation and care plans are current and compliant.

The Assessment Nurse supports long-term care residents by facilitating appropriate assessment, evaluation and action needed to improve quality of care according to evidenced-based practices. This individual identifies risks and incorporates appropriate approaches, documentation and monitors of processes and care. The Assessment Nurse works closely with teams on each unit of Lake Winona Manor as a mentor and facilitator for the delivery and execution of the care plan process.
