Job summary

Avamere Skilled Advisors, LLC d/b/a Avamere Living is seeking a Medication Tech for Avamere at Chestnut Lane in Gresham, OR. This PRN / On-Call role requires compassionate care in a Deaf/DeafBlind community with varied shifts.

You will administer medications and provide direct care to residents, documenting in EMR and coordinating with the nursing team to meet safety and quality standards.

Qualifications

• Experience as a Caregiver and Medication Tech in Memory Care or Assisted Living.
• Ability to obtain CPR/First Aid certification during employment.
• Ability to obtain a Food Handler's Card.
• Experience with electronic medical records documentation.
• Must be 18 years or older.
• Friendly, attentive, and professional demeanor with residents.

Responsibilities

• Pass medications according to physician orders, delegated by and under supervision of a licensed nurse.
• Administer accurately and timely.
• Record medications administered in the resident's record.
• Notify the pharmacy (or family when applicable) when medications need to be refilled.
• Maintain a clean and organized medication cart and room.
• Monitor and report side effects of medications to the Director of Health Services.
• Obtain/provide shift reports and document changes to medication orders.
• Follow care plans based on residents' needs and assessments.
• Assist with medical appointments and advocate for residents' needs.
• Observe residents for health changes and report concerns.
• Document changes in residents' status in EMR.
• Complete accident/incidents reports timely.
• Perform delegated nursing services as assigned by the RN.
• Participate in in-service education programs.
• Report changes in resident conditions to the Director of Health Services.
