Job summary

Talent With Heart seeks an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based health organization in Washington. Working within the Patient Centered Medical Home model, you will collaborate with physicians, PAs, ARNPs and nursing staff to deliver high-quality care across a diverse patient population.

Qualifications

• ARNP program graduate from an accredited program.
• Current RN and ARNP licensure in Washington state.
• Prescriptive authority in Washington state.
• Experience in broad primary care clinic settings.
• Experience with minor surgical techniques is preferred.

Responsibilities

• Perform advanced clinical assessments and manage illnesses within ARNP scope.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care.
• Develop and implement teaching plans to support patients’ health.
• Use independent clinical judgment based on evidence-based data.
• Maintain chart documentation within a 48-hour timeline (95% of the time).
• Meet productivity expectations (3,400 encounters per year full-time).
• Provide care coordination through daily huddles in the PCMH model.
• Identify at-risk populations and monitor panel-specific health.
• Empower patients through self-management education and discharge planning.
• Explain procedures and treatment plans to patients before execution.
• Support clinical education of students and other health professionals.
• Maintain required annual trainings (HIPAA, Diversity, BBP, Harassment).
