Job summary

Essenti Talent seeks an experienced Advanced Registered Nurse Practitioner (ARNP) to join a patient-centered, community-based healthcare organization in Washington. This permanent, full-time position operates within a PCMH model and collaborates with physicians, PAs, ARNPs, and nursing staff to deliver comprehensive care.

You will perform advanced clinical assessments, manage illnesses, provide patient education, and ensure timely charting.

Qualifications

• Experienced ARNP with primary care exposure.
• WA ARNP and RN licensure must be current.
• Prescriptive authority in Washington required.
• Experience in patient education and care coordination.

Responsibilities

• Perform advanced clinical assessments and manage illnesses within ARNP scope.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on care management.
• Develop and implement individualized teaching plans for patients.
• Maintain timely chart documentation (within 48-hour timeline, 95% of the time).
• Meet productivity expectations (minimum 3,400 encounters per year, full-time).
• Provide care coordination through daily huddles as part of the PCMH model.
• Identify at-risk populations and monitor panel health.
• Explain procedures and treatment plans to patients prior to execution.
• Support clinical education of students and staff.
