Job summary

Markent Personnel seeks an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based healthcare organization in Washington. The role operates within the PCMH model, collaborating with physicians, PAs, ARNPs, and nursing staff to deliver high-quality care across a diverse patient population.

You will perform advanced clinical assessments, manage illnesses, and support patient education while maintaining timely charting and meeting productivity targets.

Qualifications

• Experience in broad primary care clinic setting.
• Strong knowledge of nursing process including assessment, planning, implementation, and evaluation.
• Excellent communication and patient education skills.
• Ability to work in a collaborative, PCMH-style team.

Responsibilities

• Perform advanced clinical assessments and manage illnesses and conditions within ARNP scope of practice.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management.
• Develop and implement individualized teaching plans to support patients in achieving and maintaining optimal health.
• Use evidence-based data to inform patient care decisions.
• Maintain timely and accurate chart documentation (within 48-hour timeline, 95% of the time).
• Meet productivity expectations (minimum 3,400 encounters per year, full-time equivalent).
• Provide care coordination through daily huddles as part of the PCMH model.
• Identify at-risk patient populations and monitor panel-specific population health.
