Job summary

True North Consultants, Inc. is seeking an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based health organization in Washington.

This full-time, permanent role operates within a team-based care Model and emphasizes primary care in a PCMH setting. You will perform advanced clinical assessments, manage illnesses, coordinate care with physicians and PAs, and provide self-management education.

Qualifications

• Graduate of an accredited ARNP program (required).
• Current RN and ARNP licensure in the State of Washington (required).
• Prescriptive authority from Washington State (required).
• Experience practicing in a broad primary care clinic setting (required).
• Experience in minor surgical techniques (preferred).

Responsibilities

• Perform advanced clinical assessments and manage illnesses within ARNP scope.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management.
• Develop and implement individualized teaching plans for patient health.
• Use independent clinical judgment in care decisions based on evidence-based data.
• Maintain timely chart documentation (within 48 hours, 95% of the time).
• Complete fee slips with accurate diagnosis and procedure codes.
• Meet productivity expectations (minimum 3,400 encounters/year).
• Provide care coordination through daily huddles in PCMH model.
• Identify at-risk populations and monitor population health.
• Educate patients for self-management and discharge planning.
• Explain procedures and treatment plans to patients before execution.
• Participate in quality management and peer review.
• Support clinical education of students and staff.
• Maintain required annual training (HIPAA, Cultural Diversity, BBP, Sexual Harassment).
