Job summary

Clear Concepts, Inc. is seeking an Advanced Registered Nurse Practitioner (ARNP) in Washington to join a patient-centered, community-based health organization operating within the PCMH model.

You will work with physicians, PAs, ARNPs, and nurses to provide comprehensive care across a diverse patient population. The role requires WA ARNP licensure with prescriptive authority and experience in broad primary care.

Qualifications

• ARNP program graduate with WA licensure and prescriptive authority.
• Experience in broad primary care clinic settings.
• Ability to collaborate with physicians, PAs and nursing staff.
• Strong clinical documentation and patient teaching skills.
• Commitment to PCMH model and team-based care.

Responsibilities

• Perform advanced clinical assessments within ARNP scope.
• Collaborate with care team on patient management.
• Develop and implement individualized teaching plans.
• Use clinical judgment based on evidence-based data.
• Maintain timely chart documentation (within 48 hours, 95%).
• Complete fee slips with accurate codes.
• Meet productivity: 3,400 encounters/year FT.
• Provide care coordination via daily huddles.
• Identify at-risk populations and monitor health panels.
• Support discharge planning and patient self-management.
• Explain procedures and treatment plans to patients.
• Participate in quality, peer review, and education.
• Support clinical education of students and staff.
