Top Echelon LLC is seeking an experienced Advanced Registered Nurse Practitioner to join our patient-centered, community-based care team in Washington. This permanent, full-time role operates within the PCMH model and collaborates with physicians, PAs, ARNPs, and nursing staff to deliver comprehensive care to a diverse patient population.

Qualifications

• Must be an ARNP graduate licensed in Washington state.
• Current RN and ARNP licensure in WA is required.
• Prescriptive authority in WA is required.
• Experience in a broad primary care clinic setting is required.
• Minor surgical techniques experience is preferred.
• Strong nursing process knowledge including assessment, planning, implementation, and evaluation.

Responsibilities

• Perform advanced clinical assessments and manage illnesses within ARNP scope.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care.
• Develop and implement individualized teaching plans for patient health.
• Maintain timely chart documentation (within 48 hours, 95% of the time).
• Meet productivity: 3,400 encounters per year full-time.
• Provide care coordination through daily huddles in the PCMH model.
• Identify at-risk populations and monitor panel health.
• Support clinical education of students and staff.
