Job summary

LRSolutions, LLC is seeking an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based healthcare organization in Washington. This role operates within the Patient Centered Medical Home model, collaborating with physicians, PAs, ARNPs, and nursing staff to deliver high-quality, comprehensive care across a diverse patient population.

The ARNP will perform advanced assessments, coordinate care, educate patients, and maintain accurate documentation.

Qualifications

• Graduate of an accredited ARNP program (required).
• Current RN and ARNP licensure in the State of Washington (required).
• Prescriptive authority from Washington State (required).
• Experience practicing in a broad primary care clinic setting (required).
• Experience in minor surgical techniques (preferred).
• Strong knowledge of the nursing process including assessment, planning, implementation, and evaluation.

Responsibilities

• Perform advanced clinical assessments and manage illnesses within ARNP scope of practice.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management.
• Develop and implement individualized teaching plans to support patients in achieving health goals.
• Use independent clinical judgment in patient care decisions based on evidence-based data.
• Maintain timely chart documentation (within 48-hour timeline, 95% of the time).
• Complete fee slips with appropriately matching diagnosis and procedure codes.
• Meet productivity expectations (minimum 3,400 encounters per year, full-time equivalent).
• Provide care coordination through daily huddles as part of the PCMH model.
• Identify at-risk patient populations and monitor panel-specific population health.
• Empower patients through self-management education and discharge planning.
• Advocate for patients by explaining all procedures and treatment plans prior to execution.
• Participate in quality management, peer review, and departmental practice development.
• Support clinical education of other health professionals including student nurse practitioners.
• Maintain all required annual training (HIPAA, Cultural Diversity, BBP, Sexual Harassment).
