Job summary

LRSolutions, LLC in Washington is seeking an Advanced Registered Nurse Practitioner to deliver comprehensive primary care in a patient-centered clinic operating within the PCMH model.

You will collaborate with physicians, PAs, ARNPs, and nursing staff, perform advanced assessments, and develop teaching plans to support patient health goals. License and prescriptive authority in WA are required, with relocation assistance available.

Qualifications

• Graduate of an accredited ARNP program.
• Current RN and ARNP licensure in the State of Washington.
• Prescriptive authority from Washington State.
• Experience practicing in a broad primary care clinic setting.
• Experience in minor surgical techniques preferred.

Responsibilities

• Perform advanced clinical assessments and manage illnesses within ARNP scope of practice.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management.
• Develop and implement individualized teaching plans to support patients in achieving health goals.
• Maintain timely chart documentation (within 48-hour timeline, 95% of the time).
• Meet productivity expectations (minimum 3,400 encounters per year, full-time).
• Provide care coordination through daily huddles as part of the PCMH model.
• Identify at-risk patient populations and monitor panel-specific population health.
• Empower patients through self-management education and discharge planning.
• Advocate for patients by explaining procedures and treatment plans.
• Participate in quality management, peer review, and departmental practice development.
