Job summary

The position requires PharmD, active license, and ambulatory care experience. Travel up to 50% may be required; compensation includes competitive pay and benefits.

Qualifications

• PharmD degree required.
• Valid pharmacist license in good standing.
• Ambulatory care residency (BCACP) preferred.
• Minimum two years in primary or ambulatory care.
• Immunization and MTM certification a plus.
• BLS certification within first 90 days of employment.

Responsibilities

• Collaborate with providers to adjust therapies per protocol.
• Conduct comprehensive med reviews addressing safety, efficacy, cost and adherence.
• Manage chronic diseases like diabetes, hypertension, anticoagulation, and asthma.
• Coordinate transitions of care and prevent readmissions.
• Counsel patients on medication use and adherence.
• Order, interpret labs to monitor therapy.
• Follow care protocols with independent adjustments when allowed.
• Support physician consultations and quality initiatives.
