Qualifications

• Doctorate of Pharmacy (PharmD) is required.
• Active pharmacist license in good standing is required.
• RPh certification is required; PharmD preferred.
• Residency or ambulatory care cert is preferred.

Responsibilities

• Medication management and optimization with protocol-driven framework.
• Comprehensive medication reviews focusing on safety, efficacy and adherence.
• Chronic disease management for diabetes, hypertension, anticoagulation, asthma.
• Care coordination and transitions to prevent readmissions.
• Patient education and engagement on medication use and adherence.
• Lab testing and monitoring related to therapies.
