Job summary

ChenMed LLC is seeking an Ambulatory Pharmacist to optimize medication therapy within primary care clinics. This role operates under a collaborative practice agreement to reduce polypharmacy, improve adherence, and manage chronic diseases for better CMS Star and HEDIS scores.

The pharmacist collaborates with physicians, APPs, nurses, and care teams to deliver evidence-based, individualized care that emphasizes safety, efficacy, and cost-effectiveness. Travel up to 50% locally may be required.

Qualifications

• PharmD required and licensed to practice in good standing.
• Residency or ambulatory care certification preferred.
• Immunization and MTM certification a plus.
• Minimum two years of relevant clinical experience in primary or ambulatory care.

Responsibilities

• Collaborates with healthcare providers to optimize medication therapies within protocols.
• Performs Comprehensive Medication Reviews evaluating safety, efficacy, cost and adherence barriers.
• Manages chronic diseases such as diabetes, hypertension, anticoagulation, and asthma.
