Job summary

ChenMed is seeking an Ambulatory Pharmacist to optimize medication therapy within primary care clinics. This patient-centered role operates under a collaborative practice agreement to reduce polypharmacy and improve adherence, contributing to CMS Star Ratings and HEDIS scores.

The pharmacist will partner with physicians, nurses, and care teams to deliver evidence-based, individualized care emphasizing safety and cost-effectiveness.

Qualifications

PharmD required with valid license in good standing.
Residency in ambulatory care or BCACP preferred; managed care experience a plus.
BLS certification from AMA/Red Cross within 90 days of employment.

Responsibilities

Collaborate with physicians and care teams to optimize medication therapy under protocols.
Conduct comprehensive medication reviews addressing safety, efficacy, and adherence.
Manage chronic diseases and coordinate transitions of care to prevent readmissions.
Educate patients on medication use, monitoring, and adherence strategies.
Order and interpret labs to monitor safety and effectiveness of therapies.
