Job summary

The Ambulatory Pharmacist at ChenMed LLC focuses on optimizing medication therapy in primary care clinics under a collaborative practice agreement. The role emphasizes reducing polypharmacy, improving adherence, and managing chronic diseases to boost CMS Star Ratings and HEDIS scores.

You will partner with patients, physicians, nurses, and care teams to deliver evidence-based, individualized care that promotes safety, efficacy, and cost-effectiveness in a dynamic ambulatory setting.

Qualifications

• PharmD degree and valid pharmacy license required.
• Minimum 2 years of clinical experience in primary/ambulatory care.
• BLS certification within first 90 days; immunization and MTM certifications are a plus.

Responsibilities

• Medication management & optimization with protocol-driven decisions.
• Comprehensive medication reviews focusing on safety, efficacy, and adherence.
• Chronic disease management (diabetes, hypertension, anticoagulation, asthma).
• Care coordination and transitions to prevent readmissions and resolve med discrepancies.
• Patient education and engagement to improve adherence and outcomes.
• Lab testing & monitoring to ensure safety and effectiveness.
• Collaborative practice with physicians to adjust medications as needed.
• Administrative support and participation in quality improvement initiatives.
