Job summary

ChenMed, LLC is seeking an Ambulatory Pharmacist to optimize med therapy in primary care clinics. This clinician works under a collaborative practice agreement to reduce polypharmacy, improve adherence, and manage chronic diseases.

The role partners with physicians, nurses, and care teams to deliver evidence-based care, improve safety and efficacy, and support quality metrics like CMS Star Ratings and HEDIS. Travel up to 50% locally; evening/weekend hours may be required.

Qualifications

• PharmD degree with valid license in good standing.
• Ambulatory care experience preferred.
• BLS certification within first 90 days of employment.

Responsibilities

• Collaborates with providers to adjust medications under a protocol-driven framework.
• Conducts comprehensive medication reviews addressing safety, efficacy, and adherence.
• Proactively manages chronic diseases to improve control and outcomes.
• Facilitates transitions of care and prevents readmissions by resolving discrepancies.
• Delivers patient education on medication use and adherence strategies.
• Orders, interprets, and uses labs to monitor safety and effectiveness.
• Works with physicians and care teams to implement care protocols.
• Supports quality improvement initiatives within the department.
• Performs other duties as assigned by management.
