Job summary

ChenMed is seeking an Ambulatory Pharmacist to optimize medication therapy in primary care clinics, operating under a collaborative practice agreement. You will reduce polypharmacy, enhance adherence, and manage chronic diseases to improve CMS Star and HEDIS metrics.

The role involves working with physicians, nurses, and care teams to deliver evidence-based, individualized care, promoting safety, efficacy, and cost-effectiveness across patients while supporting quality initiatives.

Qualifications

• PharmD required.
• Active license in good standing.
• Ambulatory care residency/certification preferred.
• BLS within 90 days of employment.

Responsibilities

• Optimize medication therapy under a protocol-driven framework.
• Conduct comprehensive medication reviews focusing on safety, efficacy, and adherence.
• Manage chronic diseases (diabetes, hypertension, anticoagulation, asthma).
• Coordinate transitions of care to prevent readmissions and resolve med discrepancies.
• Provide patient education and engagement to improve adherence.
• Order and interpret labs to monitor safety and effectiveness of therapy.
• Collaborate with physicians and care teams to follow care protocols.
