Job summary

ChenMed, LLC is seeking an Ambulatory Pharmacist to optimize medications within primary care clinics and collaborate with physicians and care teams. This role focuses on reducing polypharmacy, improving adherence, and managing chronic diseases to positively impact CMS Star Ratings and HEDIS scores.

The ambulatory pharmacist will partner with patients, providers, nurses, and care teams to deliver evidence-based, individualized care while operating under a collaborative practice agreement.

Qualifications

• PharmD degree and an active pharmacist license in good standing.
• Pharmacy residency or ambulatory care certification preferred.
• Minimum two years of clinical ambulatory care experience.
• Immunization and medication therapy management certification is a plus.
• BLS certification required within first 90 days of employment.
• Residency or ambulatory care training highly preferred.
• Experience in managed care, HMO, PBM, or MSO settings is a plus.

Responsibilities

• Collaborate with physicians to optimize medications under protocol-driven care.
• Conduct comprehensive medication reviews focusing on safety, efficacy, cost, and adherence.
• Manage chronic diseases to improve control and outcomes.
• Facilitate transitions of care and prevent readmissions.
• Educate patients on medication use, side effects and adherence strategies.
• Order and interpret lab results to monitor therapy.
• Work with care teams to implement care protocols and adjustments as needed.
• Support department quality improvement initiatives.
• Perform other duties as assigned by management.
