• Serve as single point of contact for Memorial Health Partners members; triage symptoms and route to appropriate care.
• Provide care advice, schedule same/next-day appointments, and assist with navigating the healthcare system.
• Maintain accurate records of interactions and collaborate with care teams to remove access barriers.
