Job summary

TriHealth is seeking a Nurse Navigator to guide patients through their healthcare journey from diagnosis through treatment and recovery. You will collaborate with physicians and care teams to ensure timely, appropriate care and empower patients to manage their health.

The role emphasizes comprehensive care coordination, education, and support across episodes of care, reinforcing TriHealth's patient‑centered culture and collaborative, service‑oriented environment.

Qualifications

• Associate's Degree in Nursing required.
• 2-3 years of experience in the specialty area (orthopedics, cardiac, oncology, etc.) required.
• 2-3 years of home care nursing experience preferred.

Responsibilities

• Assess clients’ activation, literacy level and self-management capabilities.
• Evaluate patient functional abilities and limitations.
• Determine if intervention is needed and coordinate care.
• Evaluate medical necessity and optimize patient outcomes.
• Serve as liaison for physician referrals and community resources.
• Support data collection and reporting for value-based contracts.
• Develop and monitor plans of care and utilize resources accordingly.
• Educate patients and families on diagnosis, treatment plan, medications, and nutrition.
