Job summary

Capital Health is seeking a Registered Nurse to support patient care coordination and concierge-style services across our hospital network in New Jersey. The role emphasizes proactive patient communication, comprehensive care planning, and seamless transitions between ambulatory and inpatient settings.

Requires a BSN (MSN preferred) and an active NJ RN license, with a minimum of three years in healthcare navigation, guest relations, or related fields.

Qualifications

• Bachelor’s in Nursing is required; Master’s in Nursing preferred.
• Three years of experience in healthcare navigation and/or concierge services, guest relations, hospitality or patient experience.
• Registered Nurse - NJ license or eligibility.
• Strong communication and HIPAA/privacy knowledge; proficient with MS Office.

Responsibilities

• Initiate proactive patient communication to assess complex care needs.
• Develop and execute individualized plans of care based on assessments.
• Serve as patient liaison and primary contact for outpatients and inpatients.
• Coordinate transitions between ambulatory and inpatient settings and schedule consultations.
