Job summary

Rural Health Group is seeking an Ambulatory Nurse (RN) to support patients across multiple clinics in northeastern North Carolina. This role focuses on clinical documentation, EHR coordination, medication refills, and care coordination to improve provider efficiency and patient outcomes.

The ideal candidate will have an active NC RN license, at least two years of outpatient or ambulatory care experience, and strong communication skills.

Qualifications

• Current NC RN licensure in good standing.
• Minimum two years of outpatient/ambulatory nursing experience.
• Experience using EHR; ability to learn eClinicalWorks quickly.
• Care coordination experience with uninsured/underserved patients preferred.
• Bilingual English/Spanish is a plus.

Responsibilities

• Respond to incoming patient calls, portal messages, and clinical inquiries.
• Assess patient concerns and route urgent issues to the appropriate provider.
• Provide patient education regarding medications, treatment plans, and follow-up care.
• Document all patient interactions in the EHR accurately.
• Process prescription refill requests per established protocols.
• Review refill eligibility, medication history, and provider instructions.
• Coordinate with pharmacies regarding prescription clarifications.
• Investigate and resolve medication access issues and prior authorizations.
• Review incoming laboratory and diagnostic reports and communicate results.
• Coordinate follow-up testing and appointments as indicated.
• Track outstanding results and ensure provider review.
• Maintain a tracking system for high-risk and abnormal test results.
• Manage and maintain patient records within the EHR.
• Complete clinical forms and medical documentation.
• Support providers with patient care coordination and workflow management.
• Monitor patient follow-up needs and identify care gaps.
