Job summary

Moda Health's RHTP Grants Management & Implementation Specialist provides grants management, technical assistance, and subject matter expertise to recipients of the Alaska Rural Health Transformation Program grant. Qualifications

• Minimum Bachelor's degree required.
• Master's degree preferred in Public Health, Public Policy, or Healthcare Administration.
• 3-5 years of experience in healthcare, public health, or health-related community-based organizations.
• 2-3 years of experience in grant management, including contract management, monitoring and reporting.
• Familiarity with NCQA PCMH and Oregon PCMH requirements and recognition processes.
• Knowledge of VBP systems and alternative payment models (APMs).

Job description

The RHTP Grants Management & Implementation Specialist provides grants management, technical assistance, and subject matter expertise to recipients of Moda Health's Alaska Rural Health Transformation Program (RHTP) grant. This role serves as the primary liaison between Moda Health, funded partners, and other stakeholders to support successful implementation of grant activities. The position is responsible for grant monitoring, compliance, and reporting. This includes ensuring adherence to federal and state requirements and supporting grantees in achieving program goals. The position will also support the development and delivery of training and technical assistance to funded health system partners to implement National Committee on Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) standards and build partner readiness to participate in advanced value-based payment (VBP) models. Working closely with Moda Health's internal grant and quality teams and external clinical and community partners, the position supports program implementation, reporting to funders, grant proposal development, and continuous quality improvement.

This is a full-time position. The ideal candidate will reside within commuting distance of our downtown Portland, Oregon office or in Alaska's Anchorage/Kenai Peninsula region. Occasional travel to Alaska and a hybrid in-office schedule is required for Portland-based candidates. This is a limited duration position contingent on continued receipt of RHTP grant funds.

Required Skills, Experience & Education:

• Minimum Bachelor's degree required.
• Master's degree in Public Health, Public Policy, or Healthcare Administration-related field preferred, or an equivalent combination of education and relevant work experience.
• 3 - 5 years of experience working in healthcare, public health, or health-related community-based organizations. Experience working in or with rural or underserved communities preferred.
• 2 - 3 years of experience in grant management, including contract management, grant monitoring and reporting, proposal development, and providing technical assistance and support to grantees.
• Familiarity with NCQA PCMH and/or Oregon's Patient-Centered Primary Care Home requirements and recognition processes.
• Demonstrated understanding of VBP systems and alternative payment models (APMs) in healthcare settings.
• Knowledge of primary care clinic functions such as case management/care coordination, use of electronic health record (EHR) systems, population health-level metrics and reporting, behavioral health integration, and collaboration with Traditional Health Workers (THWs).
• Proficiency in Microsoft Office, especially MS Excel and other reporting software.
• Strong verbal, written and interpersonal communication skills.
• Experience convening diverse stakeholders and facilitating productive virtual and in-person meetings.
• Ability to manage complex projects, track activities, and meet short and long-term project milestones.
• Ability to maintain confidentiality and project a professional business image.
