Job summary

Moda Health's RHTP Grants Management & Implementation Specialist provides grants management, technical assistance, and subject matter expertise to recipients of the Alaska Rural Health Transformation Program grant. This role serves as the primary liaison between Moda Health, funded partners, and stakeholders to support successful implementation of grant activities.

The position handles grant monitoring, compliance, and reporting, ensuring adherence to federal and state requirements.

Qualifications

• Minimum Bachelor's degree required.
• Master's degree preferred in Public Health, Public Policy, or Healthcare Administration.
• 3-5 years of experience in healthcare, public health, or health-related community-based organizations.
• 2-3 years of experience in grant management, including contract management, monitoring and reporting.
• Familiarity with NCQA PCMH and Oregon PCMH requirements and recognition processes.
• Knowledge of VBP systems and alternative payment models (APMs).

Responsibilities

• Provide grants management, monitoring, and reporting to funders.
• Develop and deliver training and technical assistance to grantees.
• Support proposal development and grant-related communications.
• Coordinate with Moda Health and partners to ensure compliance and timely milestones.
• Support continuous quality improvement and data reporting.
