Job summary

Circadia Health in Pasadena, TX on-site seeks an experienced Nurse Practitioner to blend direct patient care with AI-driven care coordination. You will coach facility teams to turn data into earlier interventions on the bedside.

The role requires hands-on patient assessments, training staff on Circadia workflows, and turning AI insights into timely actions while maintaining daily charting and documentation.

Qualifications

• Certified and licensed Nurse Practitioner in TX.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid TX driver’s license; comfortable traveling locally.
• Thrives in a dynamic startup environment.
• English required

Responsibilities

• Direct patient care: Conduct initial and follow-up assessments; adjust plans of care; coordinate with facility providers.
• Tech → bedside integration: Train staff to standardize Circadia in clinical workflows.
• Data into action: Interpret trend insights; prioritize high-risk patients; drive timely interventions.
• Close the loop: Complete documentation within the same day.
• High-volume rounding: 30+ visits/day across designated SNFs.
• Quality & product feedback: Surface field insights to improve workflows and integration.
• Mentor & collaborate: Support NP onboarding and work with clinical ops/product teams.
