Job summary

Nsight Health is seeking a detail-oriented Medical Assistant to join the Chronic Care Management team. You will perform high-volume outbound calls, guide patients through assessments, review medications, and escalate needs to nursing staff as appropriate.

You will document interactions in the clinical platform in real time, meet performance metrics, and adapt to shifting priorities while maintaining patient care quality. Training is four weeks, with a 9–6 ET schedule during training.

Qualifications

• Active MA license required.
• Strong communication and organizational skills.
• Preferred: empathy and patient teaching.
• Experience in call center or high-volume phone-based roles is beneficial.

Responsibilities

• Conduct outbound phone calls to check in on patients and address health concerns, 70 to 90 calls per day.
• Handle inbound calls, triage by clinical urgency in a fast-paced queue-based environment.
• Route non-clinical inbound calls to appropriate departments with minimal hold times.
• Document CCM care plan calls and coordinate care per program requirements.
• Collaborate with providers to coordinate patient care via telephonic communication.
