Job summary

Circadia Health is seeking a Nurse Practitioner to support both skilled nursing and Home Health AI Care programs, seeing patients first, acting on predictive signals, and coaching facility teams to intervene earlier.

In SNF settings you’ll round on new admits and flagged patients; in home health you’ll extend high‑quality care to patients at home, blending direct care with tech‑enabled workflows.

Qualifications

• WA NP license and ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid WA driver's license; comfortable traveling locally.
• Thrives in a dynamic startup environment; English required.

Responsibilities

• Direct patient care: assess residents, adjust plans, obtain consent, coordinate with facility providers.
• Tech → bedside integration: train and coach staff in clinical workflows.
• Data → action: interpret trend signals, prioritize high‑risk patients, drive timely interventions.
• Close the loop: complete documentation on the same day.
• High‑volume rounding: 25+ visits/day across SNFs and home health as applicable.
• Mentor & collaborate: support NP onboarding and partner with klinical ops/product teams.
