Job summary

Seabury Resources for Aging is seeking a Social Services Coordinator to deliver information and referral services, counseling, and outreach to older adults in Ward 5 and 6. Responsibilities include visiting dining sites, making home visits when needed, and coordinating with the Home First program to support residents.

The role requires 2+ years in social services for aging, licensure in Social Work or Counseling, and strong interviewing and listening skills.

Qualifications

Professional with at least two years of experience in social services working with aging population.
Training, education, or licensure in Social Work, Counseling, or related field.
Skilled interviewer who can elicit needed information from clients.
Active listener who understands clients' statements.
Ability to engage older adults in conversation and provide emotional support/counseling.
Strong documentation skills and use of technology to enter service data.
Knowledgeable of the Older Americans Act and local resources.
Must have access to an automobile five days a week for field visits.
Mental health experience is a plus.

Responsibilities

Provide information on resources in the community, including assistive technology.
Facilitate referrals to senior services programs via DACL.
Provide support, resources, and guidance to older adults.
Visit Ward 5 and 6 dining sites at least twice weekly for outreach sessions.
Attend community events as requested by the Director.
Establish in-office hours for appointments and walk-ins one day a week.
Coordinate with the Home First Program Administrator for social service support.
