Job summary

Peninsula Agency on Aging, Inc. seeks a Resource Coordinator to identify, assess, advocate for, and assist individuals age 60+ and their caregivers.

This role serves as a single entry point for agency and community services and uses skilled interviewing to assess client needs. The position requires a degree in social work or related field (or equivalent experience) and at least one year of experience with the 60+ population, plus excellent communication and a solid knowledge of available

Qualifications

• Education: Degree in social work or related field or equivalent experience.
• Minimum of one year experience working with 60+ population.
• Skilled interviewing techniques.
• Excellent oral and written communication skills.
• Comprehensive knowledge of programs and services for persons 60+ and caregivers.
• Knowledge of changing eligibility requirements for these programs.
• Ability to establish and maintain effective working relationships.
• Ability to prepare reports and enter client data into agency database.
• Ability to communicate effectively both orally and in writing.
• Ability to work independently.

Responsibilities

• Maintain and update resource manual for services for 60+ and caregivers.
• Handle telephone calls, walk-in visitors, and inquiries about services.
• Advocate for and assist 60+ individuals and caregivers to secure services.
• Refer clients to appropriate persons or agencies, including Adult Protective Services.
• Conduct comprehensive screenings for meals, personal care, wills, and programs.
• Address emergency and elder abuse prevention requests.
• Make community presentations about agency services.
• Attend community meetings and trainings.
• Maintain client records and prepare reports.
