Qualifications

• Strong communication and interpersonal abilities; proficient with Windows and MS Office.

Responsibilities

• Coordinate programs and activities for eligible individuals and liaison with community agencies.
• Monitor services provided by partners and document participant interactions in the system.
• Develop and maintain a Resource Directory of local service providers serving the target population.
• Organize educational events on health care, life skills, and referrals to ensure participant engagement.
• Identify partners to provide health services and screenings at participants' homes; promote age-in-place.
• Engage volunteers and maintain professional boundaries while supporting participants' independence.
