Job summary

Aging, Inc. in Virginia seeks a part-time Resource Coordinator to serve ages 60 and their caregivers. Under supervision, you will identify needs, assess and connect clients with agency and community services through skilled interviewing.

Responsibilities include maintaining up-to-date resource materials, handling inquiries by phone and walk-ins, advocating for clients, referring to appropriate agencies, conducting screenings for meals, personal care, and programs, and preparing reports.

Qualifications

• Education: Degree in social work or related field, or equivalent experience.
• Minimum of one year experience working with 60 population.
• Skilled interviewing techniques.
• Excellent oral and written communication skills.
• Knowledge of programs and eligibility requirements for aging services.
• Ability to establish and maintain relationships with clients, professionals, and agencies.
• Ability to prepare reports and enter client data into agency database.
• Ability to work independently.

Responsibilities

• Maintain and update resource manual for services for persons 60 and caregivers.
• Accept calls, walk-ins, and inquiries about services.
• Advocate for clients and caregivers to obtain needed services.
• Refer clients to appropriate agencies, including Adult Protective Services.
• Conduct screenings for home-delivered meals, personal care, wills, and programs.
• Address emergency and elder abuse prevention requests.
• Deliver community presentations about agency services.
• Attend meetings and trainings; perform other duties as requested.
• Maintain written and electronic client records and prepare reports.
