Job summary

Metropolitan Area Agency on Aging is seeking a full-time Resource Coordinator to support older adults and caregivers. The role conducts in-person and phone assessments, provides options counseling on long-term care, and assists with care planning and transitions after hospital stays.

Qualifications

  • Bachelor's degree in a human services-related field and one year of professional or internship experience working with older adults; OR
  • Two-year certificate or degree in a human services-related field with three years of experience working with older adults; OR
  • Five years of relevant professional experience in care coordination and case management in settings such as nursing facilities, assisted living, hospitals or home care.

Responsibilities

  • Provide person-centered support to older adults and caregivers through assessments and options counseling.
  • Coordinate services and referrals with healthcare providers and community agencies, advocating for clients to ensure access to supports.

Education

A DAY IN THE LIFE OF A RESOURCE SPECIALIST

In this position, the Resource Coordinator provides person‑centered support to older adults and caregivers by completing in‑person and phone assessments, offering options counseling on long‑term care services, benefits, housing, and financial resources, and assisting individuals and families with care planning and transitions following hospital, rehabilitation, or nursing facility stays. The role involves coordinating services and referrals with healthcare providers and community agencies, advocating for clients to ensure access to appropriate supports, and managing an ongoing caseload with accurate documentation of assessments, plans, and services to promote successful community living.

MINIMUM QUALIFICATIONS
  • Bachelor's degree from an accredited college or university in a human services-related field, such as social work, nursing, public administration or gerontology and one-year of professional or internship experience working with older adults; OR
  • Two-year certificate or degree in a human services-related field or a community health certificate with three-years of experience working with older adults; OR
  • Five-years of relevant professional experience in care coordination and case management at a nursing facility, assisted living, hospital, home care or lead agency; AND
  • Knowledge of aging services, community resources, and long‑term supports
WORK SCHEDULE FOR RESOURCE COORDINATOR

This position works Monday - Friday, 8:00 AM - 4:30 PM, with two in‑office days per week and the remaining time spent working from home or in the community.

When working offsite, employees must meet specific internet criteria, including demonstrated high‑speed connectivity and the ability to meet privacy and security standards with secure (password-protected) internet and demonstrate successful performance.
