Job summary

Aging and Disability Resource Center (ADRC) serves as a single point entry for older adults and people with disabilities seeking information and access to publicly funded, community-based long-term care programs. The ADRC Specialist provides information, assistance, and counseling on long-term care options, plus functional eligibility and enrollment support.

The role requires strong communication, advocacy, and case management skills, with attention to confidentiality and program guidelines,

Qualifications

• Excellent active listening, interviewing, and advocacy skills.
• Knowledge of case work principles, long-term care, social service delivery systems and community resources.
• Proficient experience with computers.

Responsibilities

• Provide information and assistance about long-term care resources.
• Maintain up-to-date knowledge of community resources, benefit programs, and guidelines.
• Obtain and maintain Inform USA certification within 1 year of employment.
• Advise individuals and families about long-term care options.
• Maintain eligibility and enrollment documentation and participate in staffing.
