Job summary

Walworth County’s Aging & Disability Resource Center seeks a dedicated professional to provide information, eligibility determinations, and comprehensive case management for older adults and families under the Older Americans Act. This role also represents caregiver support programs within the division.

The position emphasizes coordination with providers and teams, ensuring timely access to services while maintaining compliance with state and federal standards.

Qualifications

• Master’s degree in social work or related field, and at least one year of relevant prior experience.
• Knowledge of Trauma Informed Care and Health Equity Principles is essential.
• Must be able to provide a reliable personal vehicle for business travel.

Responsibilities

• Conduct comprehensive initial assessments at consumer admission into Older American Act funded programs and update as required.
• Provide case management for assigned caseload with face-to-face contact in home or community.
• Evaluate psychosocial/physical challenges, develop service plans, implement and monitor services.
• Coordinate with contracted providers and monitor purchased services for ongoing need and effectiveness.
• Consult, coach and collaborate with families, providers and staff across programs.
• Ensure coordination of care with Aging & Disability Resource Center and Adult Protective Services units.
