Job summary

Hilltop Community Resources is seeking an ADRC Navigator in Western Colorado to provide case management, option counseling, and help elderly and disabled clients access long-term care programs. You will support outreach and coordinate with community agencies.

You will manage a caseload, conduct assessments, develop service plans, and assist with program applications while collaborating with partners to ensure seamless service delivery.

Qualifications

• Bachelor’s degree in behavioral science, human services, or a related field.
• Equivalent combination of education and experience may be considered.

Responsibilities

• Manage a caseload of 30 clients with intake and referrals.
• Interview clients and families to assess long-term care needs and options.
• Develop service plans and coordinate with providers and services.
• Assist with filling out applications for programs and ensure connections.
• Collaborate with community partners and agencies to leverage resources.
• Maintain accurate records in the State’s database and ensure documentation.

Education

Bachelor’s degree in behavioral science, human services, or a related field
