Oregon Cascades West Council of Governments seeks an Information and Referral Specialist to assist aging and disability consumers with information, referrals, and service coordination in the three-county area. The role includes phone contacts, in-home visits, and maintaining accurate service records.

The position emphasizes customer service, collaboration with medical and community resources, and adherence to privacy and reporting requirements. Office-based with travel as needed.

Formación

• Bachelor’s degree preferred, or associate degree with 3 years in social services and related customer relations.
• Experience with information and referral or call management is desirable.
• Ability to document and maintain records in ADRC/related systems.

Responsabilidades

• Provide information and referrals to OCWCOG programs and community services.
• Screen and interview consumers by phone or in home visits to identify needs and services.
• Document calls and interactions in appropriate state and internal systems.
• Coordinate with service providers and family members to arrange assistance.
