Job summary

Walworth County Aging & Disability Resource Center seeks a qualified professional to provide information, options counseling, enrollment/disenrollment counseling, and advocacy for older adults, adults with disabilities, and youth. The role includes assessing needs, determining eligibility, and coordinating long-term care services to support consumer goals.

Qualifications

• Bachelor's degree in social work or a related field and at least one year of relevant prior experience required.
• Master's degree preferred.
• Knowledge of long-term care programs such as Family Care, IRIS, AFCSP, SSIE, and MA covered services preferred.
• AIRS Certification for I & R Specialist in Aging/Disability required within one year of hire.

Responsibilities

• Facilitate consumer access to services and collaborate with contracted providers.
• Administer mandated screens to determine program eligibility.
• Provide short-term case management, including in-home or community visits.
• Complete annual protective placement reviews for persons in long-term care programs.
• Advocate for consumers and educate their support systems to achieve goals.
• Collaborate with teams to coordinate care across departments.
• Maintain required documentation and comply with standards.

Education

Bachelor's degree in social work or related field

Master's degree preferred
