Job summary

Western Reserve Area Agency on Aging is seeking a Community Resource Consultant to assess client needs, inform options, and connect individuals with home and community services across the greater Cuyahoga and surrounding counties. The role requires licensure as an RN or LSW and involves field visits, telecommuting, and coordination with caregivers and providers.

You will educate on program eligibility, document findings in compliant systems, and collaborate with internal teams to improve care

Qualifications

• Current Ohio social work licensure or active RN licensure.
• Experience in home care, geriatrics or medical social work preferred.
• Strong interviewing and documentation skills with attention to timeframes.
• Knowledge of local resources for older adults and effective communication.

Responsibilities

• Conduct face-to-face assessments in community settings to identify needs.
• Educate consumers and caregivers on eligibility for programs.
• Coordinate with healthcare providers and service systems to meet needs.
• Provide options and referrals when Medicaid Waiver is not pursued.
• Document assessments in policy-compliant software systems.
• Collaborate with internal staff and external stakeholders to improve outcomes.
• Advocate for clients when safety or risk issues arise.
• Participate in LEAN projects and training; may cover other duties as assigned.
