Northwest Regional Council (NWRC) is seeking an Aging & Disability Resources Program Specialist to connect clients with services and resources through partnerships with social service agencies, medical providers, and community partners. You will screen clients for services, make referrals, and provide general assistance to families navigating aging and disability resources.

The role emphasizes strong communication, documentation, and the ability to work with diverse populations in a hybrid work

Qualifications

• Bachelor's Degree in human services or related field and two years of volunteer or paid experience OR
• Two years relevant college courses and four years of volunteer or paid human services experience
• Equivalent combination of education and experience may be considered.

Responsabilités

• Provide information on services, products and resources to clients and families through social service networks.
• Screen clients for services and make referrals to appropriate resources.
• Offer general assistance and support to clients and community partners.
