Job summary

UnitedHealth Group's Access Hospice Care, a part of LHC Group, is seeking an Afterhours Registered Nurse to deliver hospice care for patients and families after regular hours, including nights and weekends. RN licensure and reliable transportation are required.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Valid driver's license, vehicle insurance, and access to reliable transportation.
• Preferred CPR certification or ability to obtain within 90 days of hire.
• 1+ years of clinical experience.

Responsibilities

• Triage patient needs over the phone until a nurse can provide an in-person visit.
• Respond urgently to after-hours crisis calls from patients and families.
• Makes the initial nursing evaluation within 48 hours of referral.
• Perform afterhours visits including admissions, death visits, recertification and routine visits.
• Identify and reassess patient/family needs and document outcomes.
• Document problems, goals, interventions, and patient/family responses to hospice care.
