Job summary

UnitedHealth Group's Star Hospice unit, part of LHC Group, a leading post-acute care partner, seeks an Afterhours Registered Nurse to deliver comprehensive hospice care to patients and families after regular hours, including nights and weekends.

Primary responsibilities include triaging patient needs over the phone until an in-person visit, responding to afterhours crises with urgency, performing initial nursing evaluations within 48 hours of referral, and conducting afterhours admissions, death

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle.

Responsibilities

• Triage patient needs over the phone until a nurse can provide an in-person visit.
• Respond to afterhours crisis calls from patients and families with urgency.
• Perform initial nursing evaluation within 48 hours of referral.
• Conduct afterhours visits including admissions, death visits, recertifications, and routine visits.
